Ronald Moore, 72, of Springfield, was transported to Miami Valley Hospital by CareFlight after he was seriously injured at the C.J. Brown Reservoir.
Moore was pinned between his car and a concrete boat launch ramp, according to David Roorbach, public information officer for the Ohio Department of Natural Resources. The cause of the accident is under investigation.
He is listed in critical condition as of Friday morning.
One month earlier, emergency crews responded to a report of a truck in the reservoir.
The truck hit a curb too hard and skipped over it, landing in the water, according to police.
Police on scene arrested the male driver after he was uncooperative and sent him to the police station to be tested for intoxication.
He was cited for reckless driving and two of the three docks were closed.

In late June, Michael Spencer was shot by Deputy Justin Nawman in the park.
Nawman was patrolling the park when he saw Spencer with blood on his hands and neck and holding a knife.
Body cam footage showed Spencer running at one of the deputies while holding the knife in his hands.
Nawman then shot Spencer and deputies rushed to administer first aid, but he died from his injuries.
The investigation was turned over to a Clark County Grand Jury.
The grand jury decided not to indict the deputy who shot Spencer and cited self-defense in its decision.
A toxicology report indicated that Spencer had drugs in his system at the time of his death.

Before the shooting, two accidents happened in the waters of Buck Creek.
The first accident involved a Jet ski in early June.
A fisherman found Caleb Diviak, 22, of Springfield, unresponsive near a jet ski on a Saturday afternoon. The fisherman rescued Diviak by moving him to the main dock area.
Diviak was taken by CareFlight to Miami Valley Hospital, where he died two days later.
He was wearing a life jacket at the time of the rescue.
